THE KEY WORKER MODEL
It’s not about having more support.
It’s about having the right support.
Your family, therapists, educators and support workers all know different parts of you. Each person brings something valuable.
The magic happens when those people connect.
At The Connection Boutique, your Key Worker is your lead professional. They get to know you, understand what matters right now and help connect the people already in your life. They don’t replace great supports or try to be everything. They bring the pieces together and help your team work towards common goals.
One lead professional.
A whole team of knowledge.
What does a Key Worker actually do?
It depends on what you need.
Your Key Worker might work directly with you to build independence, confidence, emotional and social skills, understand your diagnosis, prepare for school, employment or adulthood, or become more confident advocating for yourself.
They can also work with your family, collaborate with therapists and educators, guide support workers and help turn professional recommendations into something that makes sense in everyday life.
When things become more complex, your Key Worker can step into advocacy and complex case management. They can bring the right people together, help work through disability-related barriers and advocate alongside you when your needs aren't being understood or met.
Sometimes advocacy also means making sure there is clear evidence behind what you are asking for. Embedding the knowledge and understanding of your professional careers, we can prepare Functional Capacity Assessments, reports or letters of support, and help provide the NDIS with a clearer picture of your needs and the supports required, bringing information from across your life and team together.
Sometimes we build skills. Sometimes we build the evidence. We always help remove the barriers getting in your way.
Less appointment burnout. More connected support.
Life can quickly become filled with appointments, particularly when several professionals are supporting the same person.
Where appropriate, your Key Worker can bring strategies from different professionals together and help embed them into everyday life. This creates more opportunities to practise and grow without every goal automatically needing another separate appointment.
Your family, educators and support workers can also be supported to understand the strategies being recommended and how to use them confidently.
Support should add to your life, not take over it.
Building capacity around you.
Good support isn't only about building your skills. Sometimes the biggest difference comes from helping the people around you understand what you need and why.
Your Key Worker can build the knowledge and confidence of families, educators, employers and support workers, creating greater consistency across home, school, work and the community.
But over time, we want that understanding to belong to you too. We want you to understand how you learn, what helps you regulate, where your strengths are, what takes more energy and what you need from the people and environments around you.
The goal is for you to become the expert on yourself.
Sometimes we're beside you. Sometimes we're in the background.
Key Worker support doesn't have to look the same every week. There may be times when you need us closely involved. You might be navigating a complex situation, preparing for a major transition, needing advocacy, building a new skill or trying to get a team that has become disconnected back on the same page. We might step in intensively, help work through what is happening and then step back again.
At other times, life might be travelling well. Your Key Worker can sit quietly in the background, keeping connected and ready to step forward when something changes.
The right level of support, at the right time, with a reason for being there.
Could a Key Worker add value to your team?
You don't need to know exactly what service you need before you contact us.
Start with a free 15-minute consultation. Tell us about you, who's already in your corner, what's working and where things aren't quite coming together.
We'll listen first. If a TCB Key Worker could add something meaningful, we'll talk with you about what that could look like. If we think another support would be better suited, we'll tell you that too.
